1.    Eligibility.  This Promotion open to residents of the State of Arizona, age 18 or older at the time of entry.  This Promotion not open to employees or immediate family members of employees (spouse, parent, siblings and/or children) of The Sidewinder Investment Group LLC (dba the Arizona Rattlers), Blue Cross Blue Shield of Arizona, the Arena One Football LLC and its Member Teams, and their respective parents, subsidiaries, affiliates, sponsors, directors, officers, governors, employees, agents and advertising and promotion agencies.

2.   Entry.  No purchase is necessary to enter this Promotion or win the prize. To participate, participants may either: (1) visit the Arizona Rattlers official website at www.azrattlers.com  and complete the online Blue Cross Blue Shield Healthy Living on-line entry form, or (2) send a 3@x5@ card with name, address, phone number, email address, age. Mail this card/form to: Arizona Rattlers, Blue Cross Blue Shield Healthy Living Contest, Attn: Tiffany Bear, P.O. Box 21717 Mesa, Arizona 85277. All entries must be received by July 23, 2010 11:59MST. Online entry will be available from June 05, 2010 through July 23, 2010 11:59 MST.  Participants may elect to receive additional information from Blue Cross Blue Shield of Arizona by checking the ASend me information from Blue Cross Blue Shield of Arizona@ box on the online entry form. Incomplete entries will be deemed void. The Arizona Rattlers are not responsible for lost, misdirected, late, postage due, incomplete, illegible or unintelligible entries, or for inaccurate entry information, whether caused by participant or by any of the equipment or programming associated with or utilized in this Promotion, or by any technical or human error that may occur in the processing of entries. Mechanically reproduced or automated entries are not permitted and will be deemed void. Entry limited to one (1) entry per person per day.

3.     Prize. Two (2) Front Row tickets to the July 30, 2010 Rattlers  vs. Iowa Barnstormers game, a signed replica Nick Davila jersey and an Arizona Rattlers gift bag with a $100 Team Shop gift card.  No transfer, assignment, cash redemption or substitution of prizes is permitted except by the Arizona Rattlers due to prize availability, and then with a prize of equal or greater value.  Prizes not won and claimed by eligible winners in accordance with these Official Rules will not be awarded and will remain the property of Arizona Rattlers and/or the Sponsor.

4.     Odds of Winning. Odds of winning depend on number of eligible entries received.

5.    Winner Notification and Acceptance.  Winner will be selected by random drawing on July 26, 2010. The total approximate retail value of each prize is $700. Winner will be contacted by phone between July 26, 2010 and July 27, 2010 and will be required to sign , within 10 days following prize notification, an Affidavit of Eligibility and Liability/Publicity Release (or prizes may be forfeited and awarded to an alternate winner). If the prize is not claimed within 24 hours of notification, or if the winner does not meet qualification criteria, an alternate winner will be selected. Winner must be available to attend the July 30, 2010 Arizona Rattlers vs. Iowa Barnstormers game.
 
6.     Taxes.  Winner is solely responsible for all applicable federal, state and local taxes and any expenses. Any additional expenses (if any) incident to the prize are the responsibility of the winner

7.     Prize Winner List.  Winners= name can be obtained by sending a self-addressed, stamped envelope to Arizona Rattlers, Blue Cross Blue Shield of Arizona Healthy Living Contest/List of Winners, Attn: Tiffany Bear, P.O. Box 21717 Mesa, AZ 85277.

8.    Limitations of Liability.  By entering this Promotion, entrant agrees that: (1) any and all disputes, claims, and causes of action arising out of or in connection with this Promotion or any prizes awarded, shall be resolved individually without resort to any form of class action, and entrant waives his or her right to a jury trial for such disputes, claims, and causes of action; (2) any claims, judgments and awards shall be limited to actual out-of-pocket costs incurred in connection with entering this Promotion specifically excluding all other costs including but not limited to attorneys= fees; and (3) under no circumstances will any entrant be permitted to obtain any award for, and entrant hereby waives all rights to claim punitive, incidental or consequential damages and any and all rights to have damages multiplied or otherwise increased and any other damages, other than damages for actual out-of-pocket expenses incurred in connection with entering this Promotion.
9.    Construction.  Participants agree that the decisions of the Arizona Rattlers related to this Promotion and all drawings are final.

10.    General Release. By participating in this Promotion, participants agree to be bound by these rules. By accepting prizes, winners agree that the Sidewinder Investment Group, LLC (d/b/a The Arizona Rattlers), Blue Cross Blue Shield of Arizona, the Arena Football One LLC and its Member Teams, the City of Phoenix, and their respective parents, subsidiaries, affiliates, sponsors, directors, officers, governors, employees and agents will have no liability and will be held harmless by winners for any liability, loss, injury or damage to property or to person, including death, due in whole or in part, directly or indirectly, by reason of the acceptance, possession, use or misuse of the prizes or participation in this Promotion, even if caused or contributed to by the negligence of the Sidewinder Investment Group, LLC (d/b/a The Arizona Rattlers), Blue Cross Blue Shield of Arizona, the Arena Football One LLC and its Member Teams, and/or the City of Phoenix.
 
11.     Additional Terms. The Arizona Rattlers reserve the right, in its sole discretion, to modify, terminate, or suspend this Promotion should viruses, bugs, unauthorized human intervention or other causes beyond the Arizona Rattlers= control corrupt or impair the administration, security or fairness of this Promotion. The Arizona Rattlers are not responsible for any human error that may occur in the processing of the entries, nor are they responsible for any error, omission, interruption, deletion, defect, delay in operation or transmission, communications line failure, theft or destruction or unauthorized access to, or alteration of, entries, whether caused by the sender or any of the equipment or programming associated with or utilized in this Promotion. The Arizona Rattlers reserve the right, in its sole discretion, to disqualify any individual found to have tampered with the entry process or the operation of this Promotion or web site; to be have acted in violation of these rules; or to have acted in an unsportsmanlike or disruptive manner or with intent to annoy, abuse, threaten or harass any other person. Any use of robotic, automatic, programmed or similar entry methods will void all such entries submitted by such methods. In the event of a dispute as to entries submitted by multiple users having the same e-mail account, the authorized subscriber of the e-mail account used to enter this Promotion at the actual time of entry will be deemed to be the participant and must comply with these rules. 12.     Miscellaneous.  Except where prohibited, participation in this Promotion constitutes participant=s consent to the publication of his or her name, biographical information and likeness in any media and to the use of their names, likenesses or voices in any media for any commercial, advertising or promotional purposes without limitation and without further consent or compensation. Winner hereby agrees and consents, without further authorization, compensation or remuneration of any kind, to the use of winner's name and/or likeness in any and all advertising promotions and other publicity conducted by the Arizona Rattlers, Arena Football One LLC and its member teams,  and each of their respective parents, subsidiaries and affiliates, except where prohibited by law.9.     Contest Void Where Prohibited. This Promotion is void where prohibited or restricted by law.  In the event that this Promotion is challenged by a legal or regulatory authority, the Arizona Rattlers reserve the right to discontinue or modify this Promotion or to disqualify participants in the affected geographic areas.  In such an event, neither the Arizona Rattlers nor the Sponsor shall have any liability to any participants that are disqualified due to such action.10.     Sponsor.
